Extract from the Personal Records Database
You are requesting an extract from the Personal Records Database (Basisregistratie Personen or BRP) from the municipality where you are registered. An extract contains your name, date of birth, place of birth, and address. Other information may also be included. Therefore, please clearly indicate the purpose for which you need the extract.
You can request your extract digitally or you can make an appointment and come to the town hall in person. Make your choice:

Make an appointment (in Dutch) Select option "Uittreksel BRP".
There are various types of extracts, such as a Proof of Being Alive (in Dutch: Bewijs van in leven zijn or attestatie de vita). A housing association may require an extract, and it may also be requested during border controls if you are traveling with a minor child or children.
The cost for an extract from the Personal Records Database (in Dutch: Basisregistratie Personen or BRP) is €15.05 (rate for 2024).
If you visit the municipal office, you can pay for the extract in cash or with a debit card at the counter, and you will receive the extract immediately.
To apply for an extract, you will need:
- Your valid identification document
If you wish to apply for an extract on behalf of someone else, you should also bring:
- A written authorization
- A copy of the valid identification document of the person for whom you are making the request.
